An Oklahoma employee benefits broker says it has notified approximately 5,500 people about a stolen company laptop with personal information from the trunk of an employee's car on April 2, 2014.

In a statement on May 21, 2014, Maschino Hudelson & Associates immediately notified police but the laptop has not been recovered.

They said the computer contained personal information such as name, address, date of birth and Social Security numbers for employees of some of its customers. In addition, the laptop may have also included some claims information.

Approximately 3,800 of the individuals who received a letter from MHA live in Oklahoma.

MHA says it takes privacy and the safety of personal information very seriously and is taking action to tighten electronic media policies so that this does not happen again.

MHA says it has offered free credit monitoring services to those impacted, adding it has no indication that any harm has occurred as a result of this incident.

MHA is headquartered in Oklahoma City and has a branch office in Tulsa.
