Silver Alert Issued For Woman Last Seen In Muskogee

A Silver Alert has been issued for a woman last seen at 8 a.m. Sunday morning in the 1300 block of South 134th Street West in Muskogee. Hazel Tate, 65, may be driving to Chicago, according to the Oklahoma Department of Public Safety.

Authorities believe Tate may be driving a 2010 Chevrolet Equinox with Illinois license plate. The tag number was not known, a news release stated.

The Muskogee County Sheriff's Office said she is a black female last seen wearing a charcoal gray hat, green and white plaid shirt and gray sweat pants.  She is under a proven medical disability and in imminent danger of serious bodily injury or death.

Her disability was not listed in the Silver Alert.
