A Muldrow woman is dead following a crash in Sequoyah County that took place earlier this month. Shelby Howard, 20, passed away Saturday morning at a Fayetteville hospital, according to the Oklahoma Highway Patrol.

Troopers say Howard was driving westbound on Highway 64 outside Roland on December 12, 2014. She lost control of her pickup and crossed into oncoming traffic, hitting a semi truck. A third vehicle was struck by a large chunk of debris, the collision report states.

Howard was ejected from her truck. The truck driver, Ernest VanDusseldorp of Hartman, Arkansas, was also injured but not seriously, troopers say. The third driver wasn't hurt.

Troopers say Howard was driving at an unsafe speed.
