Oklahoma Highway Patrol released the results of their investigation into a fatal head-on crash on the Keystone Dam. Several emergency agencies responded to the crash that took place at 6:03 a.m. Monday, November 14, 2016.
One woman was killed and a man was injured.
Troopers say Kristen Chappell was northbound on State Highway 151 in a 2003 Honda Civic when she went left of center. She hit a 2015 Chevrolet City Van driven by Cody Metcalfe of Sand Springs.
Chappell, a 22-year-old Tulsa resident, was pronounced dead at the scene by Keystone Fire Department. She suffered massive injuries, the report states.

Keystone and Tulsa firefighters worked for almost two hours to free Metcalfe, 27. He was taken to the hospital where he is said to be stable with internal and leg injuries, according to the collision report.
OHP said Chappell's condition at the time of the wreck is under investigation. Metcalfe has in apparently normal condition, troopers say.
Chappell was not wearing a seatbelt; Metcalfe was belted in, the report states.
