COLLINSVILLE, OK -- The NTSB has released its preliminary report of a fatal airplane crash near Collinsville on September 28.
Oklahoma Highway Patrol Captain Larry Jackson was killed in that accident, along with Jackson's friend, Harvey Hazelwood.
The crash occurred at a private airport, Airmen Acres.
Airport neighbors told NTSB investigators, the Cessna 320D airplane had been purchased four years ago and the flight on September 28 was the first it had been flown after being restored.
In the report, a witness at the airport was watching the airplane conduct its maintenance test flight when he heard the plane's engines running at a high rpm then return to normal.
At that point, the plane was making a left turn with landing gear down as it returned to the airport and a second later, the airplane was headed almost straight down with the wings rolling.
Both the pilot and the passenger in the plane were killed on impact.
Federal investigators are still investigating the cause of the accident.
