An investigation is underway into an industrial accident that killed a Tulsa man.

Broken Arrow Police say 54-year-old Donald Linville was operating a piece of construction equipment at the future site of the Destiny Church. The equipment fell into the orchestra pit, throwing Linville off.

He was flown to a hospital where he died.
