There's more fallout in the wake of a police department report that led to Tulsa's police chief being put on administrative leave.

Mayor Bill LaFortune placed Chief Dave Been on paid leave because he was upset with the time it took to receive an interim report that was critical of the SWAT team. As a result of the report, the acting police chief has re-assigned seven top leaders, and says more changes are to come.

During Monday's news conference Mayor LaFortune said he didn't even know a report was being done. But the News on 6, found documents dated December of last year that show when the police department requested the money for a SWAT review, one is even signed by the Mayor approving the money to hire the SWAT expert. The Mayor's chief of staff says LaFortune mis-spoke on Monday.

The mayor's office released the SWAT report to the public Tuesday, minus names and tactical operations.
