(Stillwater-AP) -- Some state tax booklets have been found with the names, addresses and Social Security numbers of Oklahoma taxpayers inside.

State Tax Commission spokeswoman Paula Ross says the mistake occurred during the printing of the booklets and quality control failed to catch it.

Ross says it appears the books with the personal information were sent only to the Stillwater Public Library, where ten were found. The mistake was discovered when a library patron picked up a state tax packet and later found the information inside.

The Tax Commission prints about 1.5-million booklets for individual taxpayers and sends about 3-million blank booklets to local libraries.
