(Tulsa-AP) -- The nursing home that an Alzheimer's patient found dead in the woods last month wandered away from has been plagued with problems since it opened, records show.
Bonita Pfeiffer was the third person in ten years to die after wandering from the home.
The nursing home has been cited by the Oklahoma Health Department for numerous violations, including not addressing the wandering behavior of residents.
Green Parks owner Stanley Arbus says the home has security measures in place, including alarms and coded doors, to prevent residence from leaving without being detected.
Recent violations at the nursing home include having dirty facilities smelling of urine and with loose and poorly affixed handrails and residents with dried blood and urine on their clothes.
