One woman is dead after a head-on collision in Tulsa County.
According to a report from the Oklahoma Highway Patrol, a utility truck and a mini van collided on 41st Street South near 157th West Avenue just before 4:00 Monday.
They said a 2000 Ford F250 towing a utility trailer loaded with a small excavator was heading west on 41st Street behind two other vehicles.
The report says the two vehicles were slowed or stopped, waiting to turn south onto 157th West Avenue when the truck, driven by 46-year-old William Gavrilov, failed to see the other vehicles.
OHP said Gavrilov tried to avoid crashing into the back of the vehicles, swerved left of center and crashed into a 2007 Honda Odyssey minivan head-on, killing the driver, 66-year-old Karen Slater.
According to the report, Gavrilov and his passenger, 30-year-old Mark Valeryevich were treated at the scene for minor injuries.
The report said neither Gavrilov nor Valeryevich had a seat belt in use but Slater was wearing her seat belt at the time of the accident.
