OKLAHOMA CITY (AP) -- The nation's largest uniform service issued a statement blaming an employee who died at a Tulsa plant for failing to follow safety procedures.

The man fell into a dryer March Sixth at Cintas' commercial laundry facility.

Cincinnati-based Cintas issued a statement that included excerpts from a letter CEO Scott Farmer sent to the company's employees.

In it, Farmer says he's grief-stricken at the death of Eleazar Torres-Gomez, but he also says that the employee climbed on top of a moving conveyor to dislodge a jam, which violates all safety training and procedures.

A spokesman with a group trying to unionize Cintas workers called on the company to take responsibility for the death. The U.S. Occupational Safety and Health Administration is investigating the accident.
